When
you call to make your appointment for your first visit, our staff will need all
of your demographic information and we will ask for your insurance information,
so that we can verify your benefits prior to your visit.
If you do not have insurance, we will happily make arrangements for self-pay.
If you have not filled out the new patient forms prior to your visit, please arrive
15 minutes earlier than your appointment time to fill out paperwork.
Things to bring with you:

- Payment for co-pay. Our office accepts personal checks with driver's license of
the account holder, as well as credit and debit cards.
If you are making an appointment for surgical consultation and you have records
from prior physicians, please bring them with you, or arrange to have records sent
prior to your visit.
